Contributor Agreement Form

“Agreement” means this Contributor Agreement Form.
“Author” means you, the contributor.
“Submission” means the story you have submitted.
“Final Work” means the final book including your Submission specified on the first page of this
“Publisher” or “Publishers” means Rob Partridge and any licensees, assignees companies connected with publishing the Final Work.

1. The entire Submission is a true account of the Authors coming out experience.
2. This word-for-word version has not been formally published in any other media.
3. It is not under consideration for publication elsewhere.
4. It has not been accepted for publication elsewhere.
5. It does not infringe any copyright or any other third party rights
6. I have not assigned, licensed, or otherwise transferred any right or interest in the Submission to anyone.
7. The Submission contains nothing that is libellous, or which would, if published, constitute a breach of contract or of confidence or of commitment given to secrecy.
8. To my knowledge all statements contained in it purporting to be facts are true

I agree to allow Rob Partridge and any of their associates to publish our Submission under the following conditions:
1. No changes are made to the Submission other than spelling corrections and layout for the Final Work.
2. Any transcribed interviews are a word for word account of any of the questions printed in the final work. (Please note: not all questions during the interview will be used in the final edition of the work)
3. Credit to me is given as requested on the previous form.
4. Only the name, age and location listed above (where applicable) is used.
5. I will not apply any legal terms or conditions once I have been informed that the Submission has been entered in the final draft of the work.

As author of the Submission sent in to Rob Partridge for publication:
1. I sign over all copyright to this Submission to Rob Partridge.
2. I grant Rob Partridge, an exclusive, worldwide, perpetual, irrevocable, royalty-free license to publish, reproduce, distribute, display and store the Submission in all forms, formats and media in which it is now or in the future capable of existence (including without limitation in print, digital and electronic form), throughout the world; to publish the Submission in the final work or any third party repositories; and translate the Submission in any language.
3. The Submission may be published in whole or in part, alone or in combination with other Submissions for promotional use but the final piece will be whole and as submitted
4. I agree that the Journal may change the appearance of the Submission (layout and format) to ensure consistency and readability.
5. I agree and understand that Rob Partridge may use a pseudonym for themselves as the coordinator and editor of the Final Work.

I hereby grant Rob Partridge the right to use quotes or statements from the Submission for use in any way, commercial or otherwise, and in any format which it is now or in the future capable of existence (including without limitation in print, digital and electronic form), throughout the world in connection with the final work, and the sale, promotion and distribution thereof.

1. That all possible efforts have been made to protect the identity of the individual(s) and/or organisation(s) mentioned in the Submission.
2. Where people can be identified I have their permission to be included in the Submission.

I certify that I have will have no financial involvement or gain from the final work or it’s promotion in any format which it is now or in the future capable of existence (including without limitation in print, digital and electronic form), throughout the world in connection with the final work, and the sale, promotion and distribution thereof.
